The Health Department of Razi University intends to implement a non-communicable diseases screening plan for faculty and non-faculty members on the occasion of Health Week, which lasts for a week from 01/02/1400. Faculty and non-faculty members have been invited to participate in this project.

The steps of the non-communicable diseases screening plan for faculty and non-faculty members are as follows:

1- Download and fill in the attached form and send it to the University Health Department at the email address: behdasht@razi.ac.ir

2- Evaluation of the forms by the doctor of the health department and coordination with the people at risk to perform the necessary tests and health recommendations (click to receive the forms)
